Job Description:
Nexus-FACTS is excited to add a part time hybrid Therapist for
its new 180 Degrees partnership ! This is a hybrid, shelter-based
position based at the Hope House Youth Shelter in Chanhassen , MN
with some work remotely at home. Hope House opened in 2015. It is a
six-bed, short-term emergency shelter for youth ages 14 –19 who are
experiencing, or at risk of experiencing, homelessness. It is the
only youth shelter located west of Highway 100 in the greater Twin
Cities. Why This Work Matters: 180 Degrees' shelters support youth
ages 12–19 who are navigating trauma, crisis, and major life
transitions. Stays are brief — often just 14–30 days — but the
impact can be life-changing. We’re looking for clinicians who bring

Schedule and Pay: Part-time evenings/weekends - 20 hours per week
Fully licensed candidates (LPCC, LMFT, LICSW): $33-38 per hour
Clinical Trainee level: $28-31 per hour In-person work in
Chanhassen, MN at the Hope House Youth Shelter This is a hybrid
position requiring in person, remote, and travel between sites

Summary : The Community-Based Therapist provides mental health
therapy to children, adolescents, adults, and families in homes,
community settings, via telehealth, and at Nexus Family Healing
offices. Using a trauma-informed, strength-based approach, the
therapist delivers individualized care through evidence-based
models like EMDR, DBT, TFCBT, and Systemic Family Therapy. Primary
responsibilities : Intake & Assessment Complete intake forms and
diagnostic assessments. Ensure timely and accurate clinical
documentation. 2. Trauma Informed Care Complete CANS assessments
using TCOM principles. Develop, update, and monitor individual
treatment plans (ITPs) based on assessments and CAT/CANS. Provide
crisis intervention and safety planning as needed. Participate in
mandatory clinical supervision and team case consultation meetings.
Participate in family collaborative intake meetings. Provide
client-centered, strength-based therapy in collaboration with
families. 3. Therapy Services Deliver individual, group, and family
therapy aligned with treatment goals. Meet direct service and
documentation requirements. Support discharge planning and
post-care coordination. 4. Case Management Coordinate referrals and
collaborate with families, schools, and providers. Attend
supervision, team meetings, and required trainings. Maintain
compliance with all clinical and regulatory standards 5. Community
Collaboration Communicate client needs with care teams, families,
and community partners. Ensure confidentiality and HIPAA
