- - - - - - - - - - -The Certified Diabetes Care and Education
Specialist (CDCES) will work directly with patients to provide
diabetes education and support with the goal of improving adherence
and health outcomes. This remote position is responsible for
consistently meeting program -metrics while also meeting quality
requirements. The CDCES position is a part-time and independent
contractor role.Primary Responsibilities:
- Provide diabetes education and support to patients via phone,
video, or digital (chat, text, email) on behalf of our customers to
improve adherence and health outcomes
- Quickly establish rapport and therapeutic alliances via phone
with a diverse population of patients
- Produce behavior change among patients using motivational
techniques
- Present online webinars
- Consistently meet program -metrics while also meeting quality
requirements
- Other duties as assignedWhat You'll Bring to The Team: -
- Must have a current certification as a Certified Diabetes Care
and Education Specialist from the National Certification Board for
Diabetes Educators (NCBDE) -
- Must be fluent in Somali to support our patient population
- Proven experience utilizing motivational interviewing to
provide positive behavior change -
- Demonstrated ability to receive constructive feedback and then
incorporate it into patient calls to drive performance
- Must have a home office for private patient conversation along
with high-speed internet
- Able to work independently and manage time efficiently -
- Must be comfortable utilizing advanced technology (Electronic
Medical Records, Microsoft Office, scripting applications, device
data, etc.) -
- Demonstrated ability to recognize patient needs and adapt
communication techniques to best serve a diverse patient
population
- Must have a flexible schedule and be able to commit to a
minimum of 20 hours per week to accommodate patients
