The role of Supervisor is to support and train Behavior
Technician staff within the Bridges Program (SH, Scattered Sites)
to provide the highest quality of care while maintaining overall
responsibility of the program in terms of licensing, goals and
Schedule: Monday - Friday 7am-3pm
Pay rate: $20-27.00/HR DOE
Key Result Areas:
2 POPS/Action Plans per resident every 6 months
0 Unwanted Discharges
0 Fines / Correction Orders
70% Staff Satisfaction
Retention: Team retention of 96% each month, 70% for the
0 Hours Outside of Allotted Office/ Admin Time.
1 Successful Transition (every 6 months)
- 75% Billable Service at your designated site each month
Job Duties and Responsibilities: (Essential Functions)
Intake new residents ensuring that a new residents folder is set
up and accurately contains 245D required documentation.
Organize existing residents folders in the areas of 245D
Manage and meet weekly with a caseload of individuals receiving
Independent Living Skills services.
Coach and develop employees that report to you as their
Review and approve weekly documentation requirements completed
by Independent Living Skill employees reporting to you.
Handle all shadowing for new employees and ensuring on-site
orientation elements are satisfactorily complete.
Review and approve timesheets for employees reporting to
Accurately enter in billable ILS time into the billing software
Cooperatively schedule, coordinate and attend routine and
specialty medical appointments, document and communicate changes
and concerns to support team members
Prepare for and facilitate an Annual Review Meeting, getting all
needed paperwork signed and appropriately filed.
Professionally represent Bridges MN to licensing, case
management, and support teams.
Maintain 245D licensing regulations, required documents and
preparing for audits.
Incident Reporting review and education.
- Cooperatively be responsible for on-call emergency weekend
staffing phone (rotating schedule)
Required Skills and Experience (Minimum requirements in terms of
educational background, work experience, licenses/certifications or
other knowledge skills and abilities).
High school diploma/ GED required.
Valid driver's license.
Ability to pass a background check with the Department of Health
and Human Services.
Reliable transportation to and from work.
No more than two minor moving violations in the past 3
No major violations (DUI, Careless, Reckless, Serious Speed,
Driving while License Suspended, etc) in the past 5 years.
Compassionate about the care and well-being of others.
Strong interpersonal communication skills; ability to
communicate with varied levels of employees.
Strong written and verbal communication skills.
Ability to establish and maintain positive and effective work
Ability to take initiative and work autonomously, as
appropriate, once an instruction is given.
- Ability to establish and contribute to a high degree of
teamwork and collaboration with diverse groups.
