To assist the Director of Social Services in coordinating and
carrying out a program of services aimed at meeting the
psycho-social needs of Catholic Eldercare Residents, and their
Responsible to complete Admission Paperwork
Identify the need for medically related social services and
assist with the provision of these services
Identify the psycho-social and emotional needs of residents
Assists with care planning
Coordinate assigned resident case load and charting.
Provide information and referral services as needed
Schedule, notify residents/representatives and participate in
resident care conferences/special meetings with the
multi-disciplinary team and provide necessary psycho-social
information specific to each resident.
Complete assigned MDS interviews. Complete social services
portion of MDS Assessments as required.
Assist director with resident room changes.
Maintain current knowledge of government regulations and
community, public and private services directly related to the
financial and psycho-social needs of residents (health insurance
Medical Assistance, PAS, ACG programs, etc.)
Respond to resident/family/staff concerns using discretion,
confidentiality and judgment to involve all necessary persons and
coordinate special care conferences when deemed necessary
Advocate for residents’ rights
Coordinate and update potential discharge plans for each
Formulate, record and update resident care plans as needed
Performs other related duties as assigned.
Social Worker licensed by the Minnesota Board of Social Work
1+ year of Social Services experience in long-term care
MDS experience within social services responsibilities,
Collabrative team member
Adaptive to changing priorities and needs
Knowledge of subject material, concepts and issues related
social services needs in long-term care industry
Ability to integrate social, medical and psychotherapeutic data
in order to diagnose, form a treatment plan, and to provide
appropriate crisis intervention and psychotherapy
Experience in participating as an integral member of an
Sensitivity to cross-cultural issues
Knowledge of county, state and federal regulations regarding
healthcare funding, vulnerable adults, etc.
Knowledge of community resources
Ability to establish and maintain effective working relationship
with patients, families, co-workers and community organizations
