Job Description:
AccentCare is seeking a dedicated Registered Nurse (RN) for our
Home Health division, providing expert nursing care in a home
setting. This position is ideal for a compassionate professional
who values personalized patient care and is eager to work in a
supportive environment focused on enhancing patient quality of
life. The successful candidate will manage patient care in
coordination with a multidisciplinary team, adapting to the unique
needs of each patient to ensure exceptional care outcomes. Conduct
initial patient assessments and develop individualized plans of
care. Administer medication and monitor patient recovery and
progress. Educate patients and their families on proper home health
care strategies and procedures. Coordinate with physicians and
other healthcare professionals to optimize care plans. Document
patient care services by charting in patient and department
records. Maintain a safe and clean working environment by complying
with procedures, rules, and regulations. Participate in ongoing
training and professional development opportunities. Proficient in
using medical software for patient care management. Strong
interpersonal and communication skills. Critical thinking and
problem-solving abilities. Ability to work independently and manage
time effectively. Medical Insurance Dental Insurance Vision
Insurance 401k Valid Registered Nurse (RN) license in the state of
practice. CPR certified. Valid driver's license and reliable
transportation.
