    • Licensed to practice as an RN in the state of Minnesota. License is active and in good standing.
    • Able to travel within geographic area serviced by the office. Valid driver's license, reliable transportation, and proof of auto insurance required.
    • Minimum of twelve (12) months experience working in an acute care or home health environment.
    • Evidence of current CPR certification.
    • Experience in Home Care to include OASIS; minimum one (1) year experience preferred.Some functions of what you would do:
      • Completes admissions and/or visits for the Home Care team.
      • Responsible for delivering nursing care to patients as directed by the RN Case Manager and the physician's plan of care.Ability to assess and determine appropriate wound care, including obtaining MD orders.
