our team at Fernbrook Family Center.Job Summary: As a Case Manager,
you'll provide targeted mental health case management services to
clients to help them gain access to medical, social, educational,
vocational, financial, and other necessary services as they relate
to the recipients mental health needs.Mental Health Practitioners,
you'll provide home and community-based skills services to clients
to improve their basic functioning.Supervisory Responsibilities:
Coordinate services with fellow Fernbrook employees but not
responsibly for their supervision.Reasonable Accommodations: We

Expected Hours of WorkFlexible hours between 7:00am 7:00pm.Travel
to various locations required.Salary staff expected to work 40-43
hours per week.Hourly staff up to 40 hours per week, with overtime
approval required.Duties and ResponsibilitiesUtilize effective
strategies to respond to client needs.Maintain accurate
documentation.Coordinate referrals and discharge planning and
engage in ongoing collaboration community support services.Meet
with the client at a frequency (must be a minimum of once every 30
days) or upon request.Coordinate and facilitate quarterly team
meetings.Advocate for resolve crises and reduce barriers.Seek out
supervisory as needed.Participate in staff meetings and clinical
supervision sessions.Complete continuing education
requirements.Uphold Fernbrook's mission and policies.Meet
productivity standards for both positions.Perform other assigned

ExperienceA Bachelors degree in one of the behavioral sciences or
related fields, including but not limited to social work,
psychology, or nursing from an accredited college or
university.Pass criminal background check.Reliable transportation
and valid driver's license for community-based rolesPreferred
Education and ExperienceExperience with diverse client
