If you are looking for a rewarding career in a supportive and dynamic environment, consider applying to be part of our team at Fernbrook Family Center.Job Summary: As a Case Manager, you'll provide targeted mental health case management services to clients to help them gain access to medical, social, educational, vocational, financial, and other necessary services as they relate to the recipients mental health needs.Mental Health Practitioners, you'll provide home and community-based skills services to clients to improve their basic functioning.Supervisory Responsibilities: Coordinate services with fellow Fernbrook employees but not responsibly for their supervision.Reasonable Accommodations: We value inclusivity and accommodation for all employees.
