essential component that initiates person-centered care by meeting
one on one with the client in their home to implement care. This
includes assessing client status and needs, developing
individualized care plans, and providing skilled interventions
aimed at achieving realistic client outcomes. The RN Case Manager
practices nursing with respect for individual, cultural and
spiritual differences. Additionally, the RN Case Manager
collaborates and communicates with all members of the health care
team to identify and create a holistic approach to care in the home
setting. Essential Duties:
- Perform initial and ongoing home care visits and evaluates
client progress.
- Develop and implement a personalized plan of care for each
client.
- Perform nursing procedures including administration of
treatments and medications, in accordance with the State Nurse
Practice Act, physician orders, and the client plan of care.
- Completes OASIS assessments per home care regulations.
- Coordinate client care with all members of the health care
interdisciplinary team.
- Consults with MD in making changes or decisions with the client
plan of care.
- Interact with clients and families to meet individual physical,
psychosocial and spiritual needs.
- Provides client and caregiver education on disease process,
medication, and treatments.
- Observe, assess and document client care accurately and timely
according to agency standards using an electronic health
record.
- Capable of using appropriate judgment and critical thinking in
decisions that require consideration of patient welfare, customer
service, regulatory compliance, and company interest.
- Supervise LPNs and HHA's.
- Complies with the requirements of Medicaid and Medicare
regulations by following policy and procedures and maintaining
competencies and knowledge base.
- Maintains appropriate professional boundaries with clients and
families; avoids significant self-disclosure; interacts with
clients and families on a professional level.
- Attends case conferences, Interdisciplinary team meetings,
Nursing meetings and other staff / agency meetings as
assigned.
- Participates in in-service education, continued clinical
education and all professional trainings required by law.
- Assists with quarterly record reviews as requested.
- Punctual for client visits and reports to work as
scheduled.
- Participate in on-call coverage.Requirements:
- A minimum of 1-year nursing experience working in a hospital,
skilled nursing facility, or home health care setting.
- Current license as a Registered Nurse in the State of
Minnesota.
- Knowledge of home health care requirements, regulations, and
protocols a plus.
- Passion and commitment to clinical excellence, quality, and
integrity.
- Excellent interpersonal communication and patient & family
engagement skills.
- Effective verbal and written communication, assessment, care
planning, teaching, clinical, and disease management skills.
- Knowledge of nursing care required by older adults and persons
with chronic/acute conditions.
- Ability to work independently and demonstrate leadership skills
for directing care activities. of LPN/unlicensed professional staff
as well as working well within a team environment.
- Effective organizational, critical thinking, problem solving,
communication, and interpersonal skills.
- Familiar with health care programs, and community
resources.
- Basic computer skills and ability to use electronic health
record system.
- Knowledge and comfort level working with technology and agency
support systems and tools.
- Ability to comply with privacy and HIPAA laws.
- Flexibility with diverse clients, cultures, and
environments.
- Promotes the agency through positive representation and
communication of its services.
- Must pass a background study with the Minnesota Department of
Human Services and negative Tuberculosis (TB) test.
- Have a valid driver's license, clean driving record and
reliable transportation to use for work driving purposes.
