experienced in nursing, with emphasis on community health
education/experience. The Hospice RN Case Manager builds from the
resources of the community to plan and direct services to meet the
needs of individual and families within their homes and
communities.Qualifications:2 years RN experience in health care
(preferably home care or hospice)Experience in customer
service/direct member contact field preferredExperience supervising
caregiversKnowledge of corporate business management, governmental
regulations, ACHC standards, and private payer practice.Graduate of
an accredited school of nursingCurrent Minnesota Nursing
